VSP Vision Provider Hub Registration

To register for vspproviderhub.com, please complete the following information.

If you are a doctor who is also the owner of the practice, please select Practice Owner for your role.

Passwords must follow the rules below:

  • Case sensitive
  • Minimum 8 characters
  • Must contain upper and lower case characters, numbers, and/or special characters.
  • Your password can't contain your name or user ID.
  • The word “password” is not permitted.
